How can I add a Follow-up Date in SWRemote?

Follow-up Date can be found on the Debrief Screen, just before the end of the Job. In order to trigger the Follow-up Date options, you most first select either the Opportunity Generated or Lead Generated checkboxes and fill in the Opportunity Code.     

To view this information in SuccessWare21, simply open the job and go to Call Progress->Job Summary->Opportunities

Both Lead Generated and Opportunity Generated, are the same thing. They just get reported back to Successware21 in different ways.
